We look forward to meeting you for your first appointment with us. In order to make your first visit as effective as possible, we ask that you prepare for your visit in a few easy ways.

Arrive 30 minutes before your scheduled appointment.

This is extremely important. At your first visit, you will have to complete some forms necessary for our records and for insurance purposes and provide any additional information the receptionist might need. After you complete this information, we must enter it into our medical records system to begin your child's medical record with us. Also, we must check insurance eligibility. All of this must be done before you see the doctor. Since this takes some time, it is necessary to arrive early. We want you to spend all of your appointment time with the doctor and not have to rush to complete this information. Also, as a courtesy to other families with appointment times after yours, we appreciate it if you do not arrive late.

Bring your child's immunization card or other vaccination history with you.

If you do not bring this information, the doctor will not be able to determine if your child's immunizations are up to date and if any are needed at that visit or for school purposes. Unfortunately, this might require that you will have to return for another visit for immunizations if any are required for school or as part of the regular vaccination schedule.

Bring any documents with your child's medical history with you

If you have your child's previous chart other relevant medical documents, please bring them to your appointment. Often, growth charts and other information, including vaccinations will be in these documents and will be very helpful to your doctor at the first visit.
